民間傳說，唐元和七年，僧人文象遇茅山老人而傳此事。李翱著《何首烏傳》：何首烏者，順州南河縣人。祖名能嗣，父名延秀。能嗣本名田兒，生而孱弱，年五十八，無妻子，常慕道術(shù)隨師在山。一日，醉臥山野，忽見有藤兩株，相去三尺余，苗蔓相交，久而解，解而又交。田兒驚訝其異，至旦遂掘其根歸。問諸人，無識者。后有山老忽來。
 
示之。答曰：“子既無嗣，其藤乃異，此恐是神之藥，何不平之？”遂杵為末，空心酒服一錢。七日而思人性，數(shù)月身體強壯，因此常服，又加至二錢。經(jīng)年舊疾皆痊，發(fā)烏容少。十年之內(nèi)，即生數(shù)男，乃更名能嗣。后與子延秀服，皆壽百六十歲。延秀生首烏。首烏服后，亦生數(shù)子，年百三十歲，發(fā)猶里。有李安其者，與首烏鄉(xiāng)里親善，竊得服，其壽亦長。遂敘其事而傳之云。
何首烏為蓼科多年生纏繞藤本植物，根細(xì)長，末端成肥大的塊根，外表紅褐色至暗褐色，生長在海拔200-3000米山谷灌叢、山坡林下、溝邊石隙。
 
    真假何首烏可從以下四面加以辨別。
1.來源：真的何首烏為蓼科植物，而假的何首烏一般都為薯蕷科植物。
2.顏色、質(zhì)地：真的何首烏表面為紅棕色或紅褐色，皺縮不平，有淺溝，切面為淺黃棕色或紅棕色，有異型維管束環(huán)列，形成云錦狀花紋，俗稱“筋” ，質(zhì)堅實；假的何首烏表面顏色灰暗，光滑，切面顏色偏白，中間無筋,質(zhì)地松脆。
3.藤莖：假的何首烏藤一般都是后來種植時，在假的何首烏中插入真的何首烏藤人工假造而成，用力拉會拉出來。
4.味：真的何首烏味苦而澀，假的何首烏味淡。
人形何首烏是指形狀似人形的何首烏，有天然形成和人工栽培兩種：野生何首烏在生長過程中遇到石塊等硬物導(dǎo)致其不能正常生長而出現(xiàn)凸凹不平的現(xiàn)象，其中酷似人形的就是天然形成的人形何首烏；根據(jù)需要制造出各種人形模具，然后將小的何首烏置于模具內(nèi)，栽于地下，長大后即成人工栽培的人形何首烏。人工栽培的人形何首烏各部位勻稱、飽滿，就像一個雕刻藝術(shù)品�，F(xiàn)在市面上很多人形何首烏多為模具制造，購買者應(yīng)提高警惕，謹(jǐn)防上當(dāng)。
