Risperidone (RISP) is an atypical anti-psychotic agent commonly used
to manage psychotic patients. One unique feature of Risperidone is that
its metabolite, 9-Hydroxyrisperidone (OH-RISP) is as active as
Risperidone, and clinically the sum of the Risperidone and 9-
Hydroxyrrisperidone plasma concentration has been used as a means
to monitor efficacy and toxicity [1]. The therapeutic concentration level
is 2-20 μg/L for Risperidone and 10-100 μg/L for 9-Hydroxyrisperidone
respectively. The symptoms of the overdose are daze, sedation, tachycardia,
hypotension and extrapyramidal effects [2].
Monitoring of RISP and OH-RISP concentrations in plasma is an
important parameter in pharmacokinetic and toxicology laboratories.
A fast and highly reproducible analytical method with low
detection limits is very important.
This paper shows the determination of the plasma concentration of
RISP/OH-RISP with reversed phase LC-MS after fast automated solid
phase extraction (SPE). The SPE sorbent used was a mixed-mode
phase which gives extra clean extracts and high recovery values.
